Instructions for the Dilute Miniature Pinscher Exam form
OPERATION BLUE-PRINTS reminds you that your participation is
very important.
Please remember to bring a Dilute Miniature Pinscher Exam
form with you when you take your Dilute dogs to the
veterinarian for any routine or not so routine visit (annual heartworm check,
shots, health certs, etc). Veterinary Dermatologist visits are a bonus,
but not mandatory. General Practitioner
DVM opinions are welcomed & encouraged as well.
If multiple veterinarians are "on duty" at your
clinic/hospital, please ask all attending veterinarians to complete a separate
form of their individual opinions.
Send original copy
(original veterinary signatures are important) to:

Keep a photocopy or
carbon copy for your records.
It will be beneficial to the study to have each of your Dilute dogs examined annually to provide a timeline in each
dog's life.
Please compel all of your Dilute puppy buyers to participate
in completing an annual form for each of their dilutes
as well.

dog's coat, preferably the trunk (body) of the dog. Close ups of the trunk are highly
desired. Photos not required, but are
encouraged & important.
